Hospital at Home is a term we will be hearing more of in the near future. It is distinguished from traditional home care by the administration of acute care in the home. Below is a link that will give you much more information and a second link with a list of 265 hospitals approved by CMS to offer hospital at home services. Included in the list are Northwestern Medicine, University of Chicago Medicine and University of Illinois Health Services here in the Chicago area.
